Requirements for Membership and Advancement

Membership

  1. Letter of intent to the Chair of Membership & Advancement

    Dr. Eric Oliver D. Sison
    Chair, Membership and Advancement Committee

  2. Copies of the following certificates:
    1. PCP
    2. PCC
    3. PPS (for pedia)
    4. Interventional Cardiology (signed by the cathlab head and training officer)
  3. List of cardiac cath cases/procedures done as primary operator during training signed by the training officer and cathlab head with the following format: date, name, age, procedures done
    1. Adult (200 diagnostics, 100 interventions)
    2. Pedia (75 diagnostics, 125 therapeutic)
  4. Verification letter from the training officer and cath lab head and census of cases of the cathlab
  5. Census of cases from the institution certified by the cardiac catheterization head and training officer
  6. Certificate of good moral character from the cathlab head or training officer and a member in good standing of PSCCI
  7. CV with picture

Advancement

Fellow

  1. Letter of intent to the Chair of Membership & Advancement
  2. List of procedures after training signed by the cathlab head with the following format: date/name/age/diagnosis/procedures done
    1. Adult (200 diagnostics, CA & HS)
    2. Pedia (200 diagnostics, HS)
  3. Verification letter from the cathlab head that the applicant performed the cases as primary operator
  4. Census of cases from the institution certified by the cardiac catheterization head and training officer
  5. Certificate of good moral character from the cathlab head and a member in good standing of PSCCI
  6. Must have actively participated in the society’s activities (lectured, moderated or acted as panelist in annual convention, consultants’ forum, and others)

Interventional Fellow

  1. Letter of intent to the Chair of Membership & Advancement
  2. List of procedures signed by the cathlab head with the following format: date/name/age/diagnosis/procedures done
    1. Adult (100 interventions)
    2. Pedia (100 interventions)
  3. Verification letter from the cathlab head that the applicant performed the cases as primary operator
  4. Census of cases from the institution certified by the cardiac catheterization head and training officer
  5. Certificate of good moral character from the cathlab head and a member in good standing of PSCCI
  6. Must have actively participated in the society’s activities (lectured, moderated or acted as panelist in annual convention, consultants’ forum, and others)
